This course provides Rehab professionals with a comprehensive overview of the most common diagnoses encountered with older adults in home health practice. It includes descriptions of each condition’s epidemiology, pathophysiology, clinical presentation, diagnosis, medical management principles, and disorder-specific therapy considerations. Current research and evidence-based principles of assessment and therapy intervention are discussed throughout the course. Therapist Tips, sample exercises, case studies, and detailed patient and provider resource lists further enrich the learning experience. The course is presented in two modules divided by diagnosis groups. Both modules of this course will be immediately useful in home health practice for Rehab professionals who work with older adults. While this course focuses on home health practice, the majority of its principles can be adapted to any practice setting. It will be a valuable learning tool for any therapy provider who works with geriatric patients. Module 1: Cardiovascular and Pulmonary Disorders covers cardiac conditions, cerebrovascular disease with a focus on stroke, and three pulmonary disorders frequently encountered in geriatric patients: pneumonia, pulmonary embolism, and chronic obstructive pulmonary disease.

Sarah R.Stillings, MA, PT, MPT, CHT

Sarah R. (Sally) Stillings is a PT and Certified Hand Therapist in Texas. She received her physical therapy degree from the University of North Carolina at Chapel Hill. Prior to becoming a therapist, Sally taught university-level writing courses and was assistant editor of The Journal of Craniomandibular Practice. In her extensive career as a PT, she has held a variety of clinical, teaching, administrative, and business roles, including general PT practitioner, upper extremity specialist, clinic manager, hand therapy clinic coordinator, continuing education (CE) instructor, and CE course administrator. She has also served as Executive Director of the Kellermann Foundation, a U.S. nonprofit organization supporting healthcare and community development in Uganda, East Africa. Her work now focuses on creating evidence-based CE materials for rehab professionals. Sally is a lifelong runner who celebrates all those who cover the ground.

After successfully completing this course, the learner should be able to:

  1. Recognize the demographic changes occurring in the U.S. population
  2. Recall the challenges of home health therapy under current reimbursement models
  3. Define each of the following conditions or diseases, and identify their epidemiology among older adults in the U.S.:

In Module 1:

  • Atherosclerotic cardiovascular disease, acute coronary syndrome, hypertension, arrhythmias, valve disease, heart failure
  • Stroke
  • Pulmonary embolism, pneumonia, chronic obstructive pulmonary disease
  1. Recall the basic pathophysiology and recognize the clinical presentation of these conditions in older adults
  2. List the modifiable and nonmodifiable risk factors of these conditions, where applicable
  3. Identify common medical and pharmacological interventions for each of these conditions
  4. Differentiate therapy considerations for each of these conditions, including contraindications and precautions to exercise
  5. Define elements of a thorough, evidence-based therapy evaluation of a patient with one or more of these conditions
  6. Choose and apply appropriate outcome measures for specific conditions and impairments
  7. Recognize the research evidence supporting evaluation tools and therapy interventions for these conditions among older adults
  8. Identify and apply evidence-based home health interventions for patients with these conditions, as described in specific case studies
  9. Recognize the best-practice principles involved in individual case scenarios
  10. Apply the material in this course in making treatment choices with individual older adults within your practice.
